Registration starts at 8:30 am
Shotgun start at 10:00 am
Rates and Registration
$175 per golfer (cost includes breakfast, green fees, driving range, cart, lunch, and two drink tickets)
$650 per foursome*
Individual registrations will be grouped together into foursomes.
Limited spots available - register early to secure a spot!
Note: Payment is required prior to the event, and please make every effort to include the names of the golfers with your registration.

Attendee Policies
CANCELLATION/REFUND/NO-SHOW POLICY
All cancellation requests must be submitted by emailing to director@healthcareleadersmn.org by September 1, 2026, to be eligible for a refund of the registration fee less a $25 processing fee. No refunds will be provided for cancellations received after September 1st, 2026. No-shows will be billed for any unpaid registration fees.
CONSENT TO USE OF PHOTOGRAPHIC IMAGES
Any person who attends an HLAMN event grants permission to HLAMN to record his or her visual/audio images, including but not limited to photographs, digital images, voices, sound or video recordings, audio clips, or accompanying written descriptions, and for HLAMN to use his or her name and such recorded material without notification for any purpose, including advertisements for future programs and HLAMN.
